Overview

A surgical navigation system is an optical guidance platform that transforms anatomical imaging (CT, MRI) into real-time instrument tracking. Passive retroreflective markers mounted on surgical probes and patient reference frames are captured by a stereoscopic infrared camera, yielding three-dimensional coordinates at dozens of frames per second. That position data is fused with preoperative anatomy to display overlay guidance on a surgical monitor, so the surgeon can navigate to targets without direct line-of-sight visualization. The system is used in orthopedic, neuro-, and spine surgery to reduce operating time, improve accuracy, and lower complication rates.

The entire system rides on a Mobile Equipment Cart that sits within the sterile field perimeter. The Optical Tracking Camera Unit hangs above the surgical site and continuously images Reference Array Trackers mounted to the patient and Instrument Tracker Assemblies in the surgeon's hand. A Workstation Console displays the registered anatomy and updates instrument position in real time.

How it works

The Optical Tracking Camera Unit is a stereo camera rig that emits infrared light through a bank of IR LED Illuminator Array. Spheres of retroreflective material (typically 4–6 mm glass beads coated with reflective paint) are clustered on reference frames and surgical instruments. When the CMOS Image Sensor sensors see the reflections, the Image Processor calculates the 3D centroid of each marker and passes those coordinates to the main Processing Unit with Software.

Before surgery, the surgeon registers the patient's anatomy—obtained from preoperative CT or MRI—to the physical patient by touching known anatomic landmarks with a Pointer Instrument Tracker while the system records their optical positions. The registration algorithm then computes the rigid transform that maps the image space to the patient space. Once registered, any instrument tracked by the optical system can be displayed relative to internal anatomy.

During the procedure, the surgeon holds a Surgical Probe Tracker or Drill Guide Tracker toward a target. The optical system continuously outputs 3D coordinates of its markers. The navigation engine deforms or rotates the preoperative images to match the Reference Array Trackers positions, then projects the instrument onto that anatomy. The Workstation Console shows the surgeon a bird's-eye view of the anatomy with the instrument path overlaid, often with automatic alerts if the path deviates from plan.

Accuracy depends on marker visibility (occlusion kills tracking), optical calibration, and registration quality. The Stereo IR Camera Pair must see all markers on an instrument for every frame; a single marker hidden by the surgeon's hand will cause a jump in reported position. High-end systems use multiple reference arrays on different parts of the patient to detect and correct for patient motion during long procedures.

Power Distribution Module provides mains power and Li-ion Cell, 18650 battery backup so the system keeps running if line power drops mid-operation. All instruments are fully sterilizable; the camera head and cart are high-level disinfected or ethylene oxide gassed between cases.
